Please read the below information carefully and TYPE your full name below to act as a signature to complete the registration process. By doing this you agree to abide by the club's Safeguarding policies listed at the start of this form, as well as the following:

understand that you will be kept informed of cricket activities at the club, for example transport details for away matches;
understand that in the event of any injury or illness, all reasonable steps will be taken to contact you and to deal with that injury/illness appropriately;
are aware that should your child(ren) play for a senior team he/she may have to share changing facilities;
confirm that when transporting players to and from matches and training, your car will have appropriate MOT, tax and insurance;
accept that when attending matches/practice nights your safety, and that of any children in your charge is your responsibility;
confirm that the child(ren) in your care will comply with the relevant rules of the game;
understand that children in school year 7 and below are not permitted to play in senior cricket;
understand that children in school year 8 can play in senior cricket, providing you and their coach/manager provide written permission;
accept that it is your responsibility to ensure your child(ren) arrives on time and is/are collected promptly at the end of training sessions and matches, and will contact the manager if you are delayed;
